Phlebotomy Registration Information

You must complete the Phlebotomy Registration Form, Health History Form, and Release Form to begin the registration process.

To complete the registration process you must pass a background check and drug screening.

Before registering for Mid’s Phlebotomy Short-Term Training, trainees are required to complete a drug screening. Trainees are not fully registered for training until results have been received.

  • Visit a medical facility, request, and complete a 10-panel Non-DOT urinalysis screening.
  • The cost of this screening is the responsibility of the trainee.

If you have prescribed medication that would result in a positive test, submit appropriate documentation such as a prescription from a healthcare provider (physician, physician’s assistant, or nurse practitioner) at the time of the test.

  • A positive test result for any substance without a valid medical explanation will void registration for Mid's Phlebotomy Short-Term Training. You will be eligible to re-apply after 90 days.

A negative diluted result requires retesting before registration can be completed for Mid's Phlebotomy Short-Term Training.

Any retesting fees are the responsibility of the trainee. Trainees will be contacted immediately if retesting is required.

Mid may require random testing at any time at the trainees’s expense.

  • It is up to the clinical site and/or instructor to determine appropriate and safe behavior, at which time mandatory alcohol and drug testing of trainees may be required.
  • Even if taking a prescribed medication, impairment in a clinical setting is not acceptable and may lead to consequences up to and including termination from the training.

Phlebotomists and phlebotomy trainees are at risk for increased exposure to certain preventable infectious diseases and other health hazards and need to be immunized properly for protection against these diseases and to prevent their spread among patients and employees in hospitals and clinics.

All phlebotomy trainees must provide proof of immunity (actual immunization documentation or titers) for the immunizations indicated below. Students should discuss their immunization status and the advisability of receiving these immunizations with their healthcare provider. Titers are acceptable.

  • A current negative TB test (annual proof of results and date read) or negative chest x-ray
  • Hepatitis B immunizations or titer
  • Measles, Mumps, Rubella (MMR) immunizations or titer
  • Tetanus, Diphtheria, Acellular (Tdap) vaccination
  • Varicella immunization or titer
  • Influenza vaccination (seasonal)
  • COVID-19 vaccination (recommended)

Mid's Phlebotomy Short-Term Training consists of nine weeks of training in a classroom, lab, and externship/clinical settings. This training often functions as a stepping stone into other healthcare-related careers, such as medical laboratory science or nursing.

Phlebotomists are healthcare workers who draw blood and prepare samples for diagnostic testing, and they function as important members of clinical teams. Responsibilities include, but are not limited to, venipuncture and blood draw micro-collection techniques. The procedures are performed on patients of all ages at clinics, hospital labs, outpatient care centers, and nursing homes.

Mid's Short-Term Training prepares you to

  • Demonstrate knowledge of the health care delivery system and medical terminology
  • Demonstrate knowledge of infection control and safety
  • Demonstrate a basic understanding of the anatomy and physiology of body systems and anatomic terminology relating major areas of the clinical laboratory to general pathologic conditions associated with the body systems
  • Demonstrate a basic understanding of age-specific or psychosocial considerations involved in the performance of phlebotomy procedures on various age groups of patients
  • Demonstrate an understanding of the importance of specimen collection and specimen integrity in the delivery of patient care
  • Demonstrate knowledge of collection equipment, various types of additives used, special precautions necessary, and substances that can interfere in the clinical analysis of blood constituents
  • Follow standard operating procedures to collect specimens via venipuncture and capillary (dermal) puncture
  • Demonstrate understanding of requisitioning, specimen transport, and specimen processing
  • Demonstrate an understanding of quality assurance and quality control in phlebotomy
  • Communicate (verbally and nonverbally) effectively and appropriately in the workplace. 

Mid's Phlebotomy Short-Term Training fee is $1,575 and includes the required textbook and workbook.

Federal Financial Aid is not available for this training. However, funding is available for qualified individuals through Michigan Works! or Michigan Rehabilitation Services to offset some or all training costs. Contact your area agency to find out if you qualify.
